CLEAR Center of Health is a state-of-the-art Integrative Medical Center located in beautiful downtown Mill Valley, California. Our unique setting with its skylights, plants and artistic elements has the ambiance of a luxurious retreat rather than a medical clinic and helps shape our healing environment.

It is important for us to review your files prior to your appointment.

If you have recent lab work (blood tests, saliva tests etc) please bring copies of those results.
HIPPA requirements mandate that copies of tests and notes from other physicians can only be released with your written request.

Bring all nutritional supplements you are currently taking.
Gather all information or bring the prescriptions bottles for medications you are currently taking or if you have taken the medication in the recent past.
We are very through in our intake process but please do not hesitate to create a list of concerns so that we may address each one.

How do I decide which doctor is right for me?

Our doctors offer a complimentary 10 minute phone consultation to discuss your case and question their individual philosophies (available for Dr.’s Chan, Mager, Massey, and Rankin).

New patient appointments costs vary depending on physician choice.
If lab work or additional testing is required there may be additional charges. Some laboratories do not offer direct insurance billing. We do not bill insurance directly, but we do provide an insurance super-bill upon request that you may send to your health insurance provider for reimbursement. All fees are due and payable at the time of your visit.

Please note that we do have a 48-hour cancellation policy. We will attempt to fill canceled appointments and if successful there will be no charge. Therefore, providing as much notice as possible will help us to accommodate your changes.  If less than 48-hours notice is given, and we are unable to fill the appointment, you are responsible for all associated charges.

Mill Valley is just 10-15 minutes north of the Golden Gate Bridge. There is one flight of stairs leading up to CLEAR Center. If you need alternative access, please let us know in advance and we will accommodate you.
